Adoption Form

The Caddo Parish Department of Animal Services and Mosquito Control sponsors a Pet Adoption program to find suitable homes for homeless animals. In most cases these animals are strays of unknown origin or animals whose owners could no longer care for them. Citizens interested in adopting one of these deserving animals should follow the steps listed below.

1. Read the information concerning adoption policies and requirements.

2. View the Dogs & Cats available for adoption and make a selection (or two).

3. Complete the Adoption Form and bring it in person to the Animal Shelter at 1500 Monty Street, Shreveport, Louisiana between the hours of 9:00 a.m. and 5:00 p.m. weekdays.
